Use the link below to go to our Consultation page and then you'll be able to access our "new patient questionnaire." These forms - which include a detailed history, adrenal, yeast, metabolic, thyroid, brain chemistry, and nutritional questionnaire - must be received by us before your scheduled consult. You may fax it to 205-879-2381 or mail it to Dr. Murphree, Corporate Center 2700 Rogers Drive, Suite 204, Homewood, AL, 35209. You may use the link under this text to pay for the consultations by secure credit card transaction. Phone consultations must be paid for in advance. (You may cancel 48 hours before your appointment for a full refund.) Please be aware that while insurance may cover a portion of an in-office visit, it will not cover a phone consultation. We look forward to hearing from you. New Patient consults typically are 45-60 minutes. Please note, we'll do everything we can to accommodate your requests for a new patient visit. However, Dr. Murphree's schedule is typically booked 6-8 weeks out and he speaks at numerous conferences each year. Yes, we do have a waiting list for those who wish to be seen sooner.
